Five years of eMIS: Digital transformation in primary care facilities

Directorate General of Family Planning (DGFP), under the Ministry of Health and Family Welfare, Government of Bangladesh introduced eMIS facility systems in its primary care facilities. The DGFP eMIS is a digital ecosystem of mobile apps and web-based applications covering all category of field level family planning workforce, their supervisors and managers.

Country wide, the DGFP manages 3,269 primary care facilities known as Union Health and Family Welfare Center (UH&FWC). These facilities have positions of providers such as Sub-Assistant Community Medical Officer (SACMO) and Family Welfare Visitor (FWV). However, in reality, there may be one or either due to vacancies. FWVs are also placed in some higher facilities like upazila health complexes to provide family planning services. The eMIS facility systems were also implemented in some of the facilities under the Directorate General of Health Services (DGHS). Besides those of DGFP, there are over 1,000 union level facilities under the DGHS known as union sub-centers (USCs) or rural dispensaries.

The catchment area for any UH&FWC is a union, a local government unit. Union level health facilities are points of first contact for many service seekers. The UHFWCs provide maternal and child care including delivery, family planning services. General patients reporting for common ailments are also served. These UHFWCs also maintain a network of satellite clinics. These clinics are usually situated in a household setting. The facility apps are used both at UHFWCs and the satellite clinics.

The providers at facilities maintain service data in registers and there are many such registers, like: ANC Register, Delivery Register, PNC Register, Birth Register, Contraceptive Pill Register, Condom Register, IUD Follow up Register, Injectable Register, General Patient Register, Logistic Register etc. All these are bunched together in one single app called facility eRegister. This eRegister is composed of  MNC eRegister, the FP eRegister, and the GP eRegister. Month-end reports are generated from facilities called MIS 3, which are aggregated at upazila level within MIS 4. MIS 3 is generated from the app and available to the supervisors, managers. The app works both online and offline. The facility apps exchange data with community systems of eMIS and data from both type of workers are integrated seamlessly.

The table below shows the coverage of facility systems in UH&FWC as well as in similar or higher facilities (as on 31 May 2020).

The FWVs also work in facilities managed by DGHS. Accordingly, the above table shows facilities managed by DGHS: 4 district hospitals, 105 Upazila Health Complexes, 80 union sub-centers, 31 rural dispensaries and 1 10-bed hospital, altogether 220.
